Case Study: April 2017
We acted for the partner and family of a man who tragically died from a brain haemorrhage after attending South Tipperary General Hospital on three separate occasions over a three-week period. Each time, he reported severe headaches, no brain imaging or neurological review was arranged.
On his final admission, he suffered a catastrophic haemorrhage that claimed his life. His death, later examined at inquest, raised serious questions about the hospital’s failure to recognise warning signs and escalate his care.
At the inquest, expert evidence confirmed that an earlier CT scan would have identified the underlying bleed and that timely intervention could have saved his life. His partner, who bravely spoke publicly, expressed deep frustration at a system that did not listen to him or take his symptoms seriously. Through Cian O’Carroll Solicitors representation, his family sought answers, accountability, and recognition of what went wrong.
This case underscores the importance of vigilance and responsiveness in emergency medicine, and our firm’s commitment to standing with families who have lost loved ones through preventable medical failures.
